Mission Hospice & Home Care is committed to providing your patients with life-limiting illnesses with a continuum of care. We provide skilled nursing, physician home visits, physical therapy, occupational therapy, medical social work for emotional support and case management, home health aides to assist with personal care, dieticians, and volunteers to assist as needed. Our team will work closely with you, the primary physician, to ensure the highest quality care for your patient, and to find the right place for care.
We have a low patient-to-nurse ratio, allowing our staff to provide the personal, individualized care for which we are known. We tailor a care plan especially for each patient, with sensitivity to cultural, religious, and personal preferences.
Our bottom line is patient care. Our well-trained volunteers and dedicated staff, including physicians who make home visits, help ensure that our patients spend their final months in comfort and with the support they need. Services are provided to patients enrolled in our Palliative Care and Hospice programs regardless of ability to pay. Our Transitions Program provides social work, nurse consultation, and volunteer support at no cost.
Patients are referred through hospitals, physicians, caregivers, and family members. Staff are also available for informational visits in hospitals, nursing homes, assisted living facilities, and private homes, for individuals and families considering hospice or palliative care. Our hospice houses in San Mateo and Redwood City offer rooms for patients who need intensive, round-the-clock symptom management that cannot be provided at home, or who need caregiver respite. Patients typically stay in the hospice houses a few weeks to a few months.
Our expert educators and clinicians also provide continuing education for healthcare providers, including training for nursing and social work students, hospice rotations for medical students, and in-services on a range of subjects for staff at care homes and assisted living facilities.
